Provides education resources, clinical management, and support for individuals with diabetes, cardiovascular conditions, chronic kidney disease, chronic obstructive pulmonary disease (COPD), asthma, and tobacco reduction, their families, health care professionals and community members. Services include ongoing education and training based on the appropriate clinical practice guidelines. Supports the development of community based injury prevention strategies, facilitates development of a regional network among stakeholders, and stimulates development of local and regional strategic safety plans.
