Home Care Nursing
Provides home care nursing delivered to clients in the community by registered nurses.

Community Rehabilitation (Occupational Therapy/Physiotherapy)
A professional service, delivered to clients in the community by rehabilitation therapists who provide assessment and treatment to ensure a client's home is suitably arranged for their needs and safety.

Health Services for Community Living
Provides non-emergency nursing, rehabilitation, dietary and dental hygiene services to adults with developmental disabilities who live in the community.

Choice in Support for Independent Living
A self-managed model of care where clients receive funds directly for the purchase of home support services. They assume full responsibility for the management, coordination and financial accountability of their services, including recruiting, hiring, training, scheduling and supervising home support workers.

Hospice Palliative Care
Provides compassionate and competent hospice palliative care services by primary care providers.

Referral criteriaTo receive Home and Community Care services, such as community nursing or physiotherapy, the client must go through a screening process to determine eligibility. Referrals can be made by the individual, a family member, hospital or family physician.
